Care Pathways are composed of the following:
1) Programs: patient education related to a specific disease condition. This content is delivered via the patient app through insights, pledges, and biometric observations informing patients of how to manage their disease, encouraging patients to make behavior change, and provide real time biometric feedback.
2) Actions: clinical notifications based on the patient’s condition that warrants further investigation by the clinician. Actions are delivered to the Care Team via the all.health clinician portal. They are accompanied with suggested interventions that recommend the next best steps to take after receiving this notification
all.health provides a generic set of care pathways for multiple disease conditions that your institution may use in its current state. However, we also understand that each institution's protocols may be slightly different, and thus the pathways may require minor adjustments to meet the needs of your patient population or clinical goals.
As a result, all.health provides “Datology as a Service (TM)” to tailor these care pathways for individual institutions. As part of this service, our team of nurses and doctors work with your care team to identify if any adjustments to language or clinical criteria is necessary, and if so work to incorporate those into your institution's care pathways.
