Estimates patient's risk for developing 50+ conditions

Note:

Features come from only all.health data (no outside claims or EHR info that's not integrated)

Reliability of risk score depends on amount of information gathered and whether patient reported surveys are answered accurately.

 

Relative Thresholding 

  • Gray = NA/Inadequate Data

  • Blue = Low Risk

  • Pink = Medium Risk

  • Red = High Risk

  • Purple = Inconclusive Risk
